Sandhills Center is a Local Management Entity-Managed Care Organization (LME-MCO) serving people in eleven counties in central North Carolina.

• The primary goal of this position is to provide Care Coordination/Care Management functions for those members, providers and associate agencies within the special group of members that are in foster care, in need of childcare coordination/management In need of assistance to transition between services

• Serve as SHC’s liaison to DHHS’s Interdepartmental Rapid Response Team (RRT)

Position can be remote or hybrid (home and office), will require travel and face to face visits

Must hold a fully independent unrestricted licensure from the appropriate NC Licensure Board for clinical treatment services. Must have unrestricted LCMHC, LCSW, or LPA. Must reside in NC.
